Parents/guardians of all children enrolled for classes at Pure Gymnastics Ltd will be required to acknowledge acceptance of the following Liability Waiver as part of the online enrollment process: 

STUDENTS WILL NOT BE ALLOWED TO REGISTER OR PARTICIPATE UNLESS THE FOLLOWING TERMS ARE ACCEPTED:

Pure Gymnastics Ltd staff members are not physicians or medical practitioners. With that in mind, I hereby release the Pure Gymnastics Ltd staff to render temporary first aid to my child/children/ward in the event of any injury or illness, and if deemed necessary by the Pure Gymnastics Ltd staff to call and seek medical help including transportation by a Pure Gymnastics Ltd staff member or it's representatives, whether paid or volunteer, to any healthcare or hospital, or calling of an ambulance for said child should the Pure Gymnastics Ltd staff deem this to be necessary.

We, the staff of Pure Gymnastics Ltd, recognise our obligations to make our students and their parents/guardians aware of the risks and hazards associated with the sport of Gymnastics, tumbling, trampolining and dance. Students may suffer injuries that are possibly minor, serious, or catastrophic. Gymnastics, tumbling, and trampolining can be dangerous and can lead to injury, paralysis and even death. Parents/Guardians should make their children/wards aware of the possibility of injury and encourage their children/wards to follow the Pure Gymnastics Ltd rules, regulations and the coaches' instructions.

Pure Gymnastics Ltd, its coaches, and other staff members will not accept responsibility for injuries sustained by any student during gymnastics, tumbling, trampolining, dance instruction, open workouts, exhibitions, competitions, or clinics in which he or she may participate or while travelling to or from an event.

With the above in mind and being fully aware of the possibility of injury involved, I consent for my child/ward or children/wards to participate in the programs offered by Pure Gymnastics Ltd. I, or my executors and other representatives, waive and release all rights and claims for damages that my child/ward or I may have against Pure Gymnastics Ltd or its representatives, whether paid or volunteered.

I also confirm that I now have and will continue to provide proper hospitalisation, health and accident insurance coverage, which I consider adequate for my child's/ward's protection and my protection.

I also understand that it is the parents'/guardians' responsibility to warn the child or children about the dangers of the above mentioned activities and possible injuries.
